چکیده

The purpose of this study was to examine the effects of 5-fluorouracil (5-FU) and mytomycine-C (MMC) on the success rate of glaucoma filtration surgery (GFS). A superior fornix-based conjunctival flap was created on the right eyes of 12 rabbits divided randomly into three equal groups termed groups 1, 2 and 3. After a small piece of sponge soaked with balanced salt solution (BSS) in group 1 (control), 5-FU (50 mg/ ml) in group 2 and MMC (0.02 mg/ ml) in group 3 was kept on the filtration sites for 5 minutes, the filtration fistula (sclerectomy) was performed and the flap was replaced. Postoperatively, BSS, 5-FU and MMC with doses of 0.1 ml were injected 4 times with certain intervals into the inferior fornixes of the same groups. The presence of the corneal edema, neovascularization and defect, appearance of the filtering bleb, states of the aqueous humor, iris, lens and changes in intraocular pressure (IOP) were observed. In one case, severe conjunctival hyperemia associated with the corneal flush and hyphema was determined. A mild corneal edema in half the members of group 1, and all the members of groups 2 and 3, was found. The IOP, which reduced sharply in all cases immediately after the GFS, reached its preoperative level approximately in 14, 28 and 51 days in groups 1, 2 and 3, respectively. There was a significant difference (P<0.001) both within and between the groups regarding the survival rate of the filtering fistula. The duration of failure of the filtration bleb was also found to be short in group 1 when compared with group 2 and in group 2 when compared with group 3. In conclusion, the success rate of the GFS was determined to be significantly higher (P<0.001) in MMC than 5-FU and in 5-FU than in the control. Additionally, it was observed that both MMC and 5-FU produced no persistent adverse effects on the eye tissues.

The use of bioerodible polymers and 5-fluorouracil in glaucoma filtration surgery.

A study was performed to examine the effect of a localized and sustained delivery of 5-fluorouracil (5-FU) on the success of glaucoma filtration surgery in 18 rabbits in a prospective, randomized, double-masked and placebo-controlled fashion. A bioerodible polyanhydride composed of bis (p-carboxyphenoxy) propane and sebacic acid was used as the drug carrier. The polymer and 5-FU (20% by weight)...

Combined treatment with bevacizumab and 5-fluorouracil attenuates the postoperative scarring response after experimental glaucoma filtration surgery.

PURPOSE This study evaluated the use of combined bevacizumab with 5-fluorouracil (5-FU) on postoperative scarring and bleb survival after experimental glaucoma filtration surgery in comparison to the agents alone. METHODS Filtration surgery was performed on 26 female New Zealand White rabbits. The rabbits were allocated to one of four treatments: 5-FU combined with bevacizumab, 5-FU alone, be...

A comparative study of the safety and efficacy effect of 5-fluorouracil or mitomycin C mounted biological delivery membranes in a rabbit model of glaucoma filtration surgery

PURPOSE To investigate the potential usage of biological delivery membranes containing mitomycin C (MMC) or 5-fluorouracil (5-FU) in the construction of glaucoma-filtering blebs, and to evaluate their safety and efficacy. METHODS Chitosan was selected as the biological membrane carrier to prepare sustained-released membranes. Twelve micrograms of 5-FU or MMC was covalently conjugated onto the...

Evaluation of anti-TGF-beta2 antibody as a new postoperative anti-scarring agent in glaucoma surgery.

PURPOSE Postoperative subconjunctival wound healing remains the commonest cause of late bleb failure after glaucoma filtration surgery. This study was undertaken to investigate whether the human monoclonal antibody that neutralizes transforming growth factor-beta2 (CAT-152; lerdelimumab) could be used as a postoperative agent to prevent scarring after glaucoma surgery and compared it with 5-flu...

Wound modulation after filtration surgery.

Filtration surgery is the standard invasive procedure for the management of intraocular pressure in advanced glaucoma. The key to a successful outcome is to modulate the normal wound healing cascade that leads to closure of the newly created aqueous outflow pathway. Antifibrotic agents such as mitomycin C and 5-fluorouracil have been increasingly used to modulate the wound healing process and i...
